Looking for specific info on companies Safety Policies re: penalizing employees regardless of fault

Tl;Dr title Talking at work yesterday, I recounted two incidents where employees were either worried about or actually in some way penalized for vehicular accidents that were clearly in no way their fault. The most recent, I tapped the bumper of a large work truck, and the driver couldn't see any damage, so he didn't want to report it because it would go on his safety record. The older one was my father. He was outside of his truck, which was properly parked and coned, and a speeding driver crossed the line and three lanes and pinned him to his truck. His very large and well known telecommunications company employer gave him the first “points” against his clean-for-twenty-years safety record. Talking at work yesterday, I recounted two incidents where employees were either worried about or actually in some way penalized for vehicular accidents that were clearly in no way their fault. The most recent, I tapped the bumper of a large work truck, and the driver couldn't see any damage, so he didn't want to report it because it would go on his safety record. His very large and well known telecommunications company employer gave him the first “points” against his clean-for-twenty-years safety record. One of my coworkers looked at me and said, “That is not true. That did not happen.” I asserted that it was in fact true, and he repeated his statement more firmly, quickly turned around and walked away. His sense of Fairness and Justice makes him believe that this is simply Not a Possibility, and it's easier for him to believe that either I or my dad or both are lying. My dad retired two years ago, so there is probably no documentation to look for. But I seriously doubt that only two companies in the world ever have had policies like this.

So, I am looking for specific examples of companies with similar policies. E. g. the employee will get written up, reported, get points on or taken off some system, or similar, for any accident, regardless of whether the employee is at fault or not. Links to company websites or pdf documents are especially useful, as you are all obviously my friends and family who are willing to lie for me 😉

Any suggestions for other subs to post in also?

This is not the first time this man has told me that mine, my family, or close friends, actual personal lived experiences Did Not Happen. I couldn't do anything about the other times, as I can't take him back in time to see/experience along with us. This time, I might actually be able to prove that his hard headed arrogance is entirely misplaced. Thank you!
