My Manager is a Dumpster Fire

My manager is a dumpster fire. Puts employees down, degrades them, gives them work then takes it away when it's at or near completion. Will sign off on deliverables and later say they're inaccurate. Recently he fabricated an impromptu and completely fallacious performance review, and worked with HR and his manager to codify it before presenting it to me. I found it slanderous, defamatory, and frankly embarrassing. HR has asked me to sign it to acknowledge receipt but I haven't done so yet. Even though the signature is to acknowledge receipt I feel that my signature could be misconstrued as me agreeing to what's written. I wrote an appeal to my manager who said he was going to talk to his manager about it. The next time we synced up he said he had not talked to his manager about it and noted that he wouldn't. I've been thinking for a while about what to do here and not sure what the best approach is. I would like the CEO, who's honestly a great guy, to understand how much of a train wreck my manager is.
