My new employers want a full medical history… RIP (just venting)

I work as a massage therapist and just accepted employment under a couple chiropractors. I love the profession as a concept, but that’s neither here nor there. I have my own chiropractor that I see as needed and I love her, but my new employers are requesting I just see them, not my current DC, for adjustments. They sent me a whole new patient form and want me to list all mental health issues, prior mental/emotional/physical trauma, what my personal birth was like, any sports injuries, any chronic or acute conditions, etc. and I think I’m going to lie. I don’t need my employer knowing I was born via c section and endured mental abuse to be able to adjust my wrists so I can massage without pain. I’m annoyed. How is this not a wild power imbalance for me to tell them all these personal, medical facts, then get…

How is this not a wild power imbalance for me to tell them all these personal, medical facts, then get on their table 3x a week and let them touch me? (They recommended 3x a week)

Like I said, I wouldn’t mind letting them just adjust my wrists since it’s no different than shaking a hand. (Although, I can see it getting to a “hmm, you’re complaining an awful lot about your wrists. Is this affecting your job performance?”) It also really helps me be more effective working there as I have a tendency to tweak my wrist in the middle of a shift, but I have my own Chiro for other issues and don’t think an employer should have their hands on an my glutes, pubic joint/symphysis, etc. even under the guise of keeping their employees in tip top shape.
