My work/no work dilemma

The time has come where I need a head gasket job done on my car which requires me to have it in the shop for a week. I’m on a very limited income and do not have any help where I live as far as rides go… I actually use my own personal vehicle to do my job in home health care. The hospital that I work for has some kind of rental “discount“ but I still have to pay money out of my own pocket to rent a car for the week… The other problem has been that my company is low census for weeks and so I’ve worked short days and supplemented there in ability to supply me with a steady caseload of patients to see in their home… so i used my PTO to make sure i got a full paycheck. My only options are: fake a 5 day illness or pay to rent a car, uber for rides to and from the damn car rental and mechanic. (I still need to uber it from/ to the mechanic)

I have plenty sick leave (EIB) and almost no PTO- i’m actually really irritated that I was forced to use PTO for the low census…. If I don’t have enough patients to see in my type of work, I am not allowed to work a full 8 hour day..

Example: you must see 5 patients in an 8 hour work day and if you see only 1-3, they start the “jedi mind trick bs talk” about how “you have really low productivity” and asking me what I was doing all day for 8 hours & only seeing a few patients.
