Should I resign from my job?

I was involved in an accident a couple of years ago resulting in 2 injuries. I was discharged from care and told the sky's the limit for employment. I passed a certification and applied for jobs. I received offers from all. I accepted my favorite and began to work. I was experiencing pain from my past injuries but decided to push through until I could receive care again. On my first day I was in tremendous pain. I took Aleve and carried on. Day two and three were the same. I rested as much as I could over the weekend. After my fourth day of my new job the pain was debilitating. I was unable to do basic needs for myself and required assistance. Informing him that my intent was not to waste people's times , that I wanted to work, and I'd be willing to fulfill any alternative role as I love the organization I work for. I was under the impression I could fulfill the role I was hired for as I was discharged from care a year ago with a clean bill of health. clearly I was not. My new boss stated he would look for another role. meanwhile, I needed to supply a doctor's note requesting an accomodation .

I called the facility I went to for the letter and was informed the provider I had seen was unable to write a letter requesting accomodations as he is an emergency level provider and I need to go to my primary care provider. I recently moved to a new state and do not have a primary care provider. I then thought I need to call my past providers as they know my history with my injury. when calling they stated I would only obtain the letter from an in-person visit. I am unable to fly back home for a letter. I called around my city to find a provider I can get in with asap. I was told there is a possibility I may not get the letter I require as they may want to go through my past medical history to confirm my injuries.

I told my employer that I had a doctors appointment tomorrow with a new provider as the provider I had seen was unable to write the note. my employer said if I am unable to submit a letter I may have to resign as they need to know my limitations from a medical source.

If I am unable to obtain the letter I would like to resign as I have been so much of a burden already meanwhile my fiance believes I should never resign they should be forced to terminate me. My fiance has asked me to post here to obtain your thought.
