Why are so many Managers/Employers/Executives (etc.), TERRIBLE at their job regardless of the field or company they occupy?

Besides the anecdotal “well I've only had great bosses,” experiences, there seems to be an overwhelming majority of employers, managers, leads, (etc.), that are terrible at what they do and do not know how to properly manage people, and/or, projects, nor do many accept criticism in stride–though have no issue delving it out themselves and expect employees to be able to take it. Why do you guys think this problem consistently presents itself in so many industries and people continue to repeat the same patterns of leadership that just don't work? Whether its a shit manager at a fast food place or a shit manager at an office, these same shitty leaders keep getting roles that they're not effective in. Just curious on other's thoughts since I'm currently dealing with said shit management and it got me thinking about previous jobs and stories I've seen from others.
