Percentage based pay rises is to blame for the ever increasing gap between workers and executives.

If I earn $10/h and my boss earns $100/h and we get a 10% pay rise. I'm now on $11/h and my boss is on $110/h. A previous pay difference of $90/h is now $99/h. Do this over many years and the rich get richer every time.
