How do I assertively ask for a raise?

My one year ‘anniversary’ with this company is coming up at the end of the month and I want to ask for a performance review and a raise. For more context, I am a server working with elders so legally we don’t get tips (while also experiencing the same stress from a regular restaurant) and i only make about .50 above minimum wage (CA). While there are many times where I’ve had to call out, be late or ask for time off (I am a full time student), I am one of the very few servers that go above and beyond, and is highly loved by the clients. We are a small team and everybody is essential. According to higher ups we’re “fully staffed” but yet there are days we struggle. If I was to quit, it would definitely be noticed and be an inconvenience to the rest of the team. I don’t mind working my ass off but I want to be compensated properly. I need help on how to assertively ask for a raise. I want to politely ask while also sounding threatening that I am not afraid of quitting on the spot if my requests/demands are not met. This job is not my life, and there are many other server jobs in my area so I have no problem walking out, I will bounce back.
