Advice for reaching out to management over gross coworker health practices

I work at a fairly small food service location. Theres one particular coworker who the vast majority of me and my other coworkers are just… over. Long story short they are a pathological liar who specializes in playing the victim and shirking all responsibility. Recently, they got hurt somehow outside of work and didn’t come in for a few days. We had heard a few different stories, but finally found out that they had an open wound that got pretty seriously infected. They came back for their first shift yesterday. They came back for their first shift yesterday. Today while working, I noticed how infected the injury really is. A while after their shift ended, a coworker came up and pointed out to me that they were hanging out in the back and rifling through the medical supply bag.

I went to the back and they were sat on the floor right beside our food freezer AND all of our personal belongings, and were rubbing some type of topical all over the uncovered wound. It was seriously gross, and 100% against Health Codes.

I need to bring this up to my manager. I want to do it in a way that drives the point home, but I am really unsure of how on earth to phrase it to them.

(I want to email the concern to my manager, always keep a paper trail)

Any suggestions appreciated!
