Doctor’s Note Requirements Preferences different between managers

So I work at a McDonald's, and told them upon applying that I had minor physical limitations. I have a protruding disc in my lower L4 S1, and the GM hired me saying that it was fine. (I cannot lift anything over 10 pounds, cannot do a lot of bending twisting, squatting movements) I asked him if I needed a doctor's note, he tells me no. Fast forward to yesterday, there was a new manager, I believe an assistant manager, (not exactly sure), but when she found out about it first time, when I told her I could not life the full to the brim ice bucket, I had asked for help, everyone was busy and they needed it (knowing this was unsanitary but unsure what else i coudl do) i started to scoot it from the icecooler to the front and she understandably got upset and asked why, and…

I told her my very next day I'd be happy to arrange that and get one for her. She said okay, soon as you can. Fast forward to today, I'm continuously already asked to do all the movements I cannot commit to already, and that's only aside from the lifting, but I'm still being asked to lift ice bags, and when I ask for help either I'm ignored or someone will say yeah, but not get to it until the customer complains to me, andi feel like its unprofessional to complain about my personal issues to a customer. How do I handle this? Please help, and legal advice or whatever would be much appreciated I don't want to hurt my back any worse.
