Bipolar and burnt-out, looking for accommodations/advice

I’m diagnosed with bipolar I and adhd. I work in controlled agriculture and it’s very fast-paced. I work about 50-55 hours a week. I love my job on paper, but it’s becoming increasingly difficult to even show up due to my disability. I’m exhausted and I just don’t feel like this current situation is sustainable. We get a half hour lunch and two fifteen minute breaks, which are strictly monitored. I feel that I can’t focus or maintain my motivation for a three and a half hour chunk of work with no breaks (for example). I’m planning on moving out of state at the end of the year, so I’m trying to ride this job out until I move. What kind of accommodations could I implement to make this a little bit easier for me? Ideally I’d like to work 40 hours a week. I’d like to be able to take a break when I need one. I really don’t feel like that’s too much to ask but my work is very … weird about this kind of thing.
