Delay in work accommodations

My position has been work-from-home since I started two years ago and a few months ago, they announced that we will be required to come into the office at least once a week. It has been documented that I am disabled, as I took FMLA exactly a year ago and have informed HR multiple times that I may be requesting work-from-home accommodations. After their announcement, a little over two months ago, I requested as needed work-from-home accommodations. I informed them of this and they said they would get back to me as they were still ironing out “exemptions”. I did not hear back for a few weeks. Finally, the director asked me to meet. She said multiple insensitive things, including that she “needs to be fair to everyone” and that once a week is not asking a lot. I clarified that it is not possible at all some weeks, especially as a new wheelchair user as the public transit system in my city is not accessible at all (I’m talking no ramps or elevators at the majority of stations). She retorted that “The public transit system is not accessible to anyone and she needs to be fair”. I told her to please let me know what information she needs to submit a formal accommodation request to her boss. She said a doctor’s note and that she’d get back to me within 5 business days after I supplied it. I supplied the note 4 weeks ago and she has ignored my follow ups. Do I have any rights here?
