Does my boss really have the authority to make me tell him why my coworker is absent?

So our boss stops by each location once a month right? And my coworker has been absent alot and the last couple of times he has been while the boss has been down. Well it's medical reasons so I really don't want to say why to him as it's not my business to say, I would expect the same respect from my coworkers. My boss pressured me to say something and I kept telling him my reason as to why (note, he has my coworker's number and could easily call and ask what's up). This last time he said he has the authority to ask and I have to answer because now I'm keeping secrets about his store. Another big reason I don't want to say anything is this boss often tells us about other people's reasons for leaving from other stores even when medical, violating PIPA. So I really feel like this decision to talk to the boss is my coworkers choice if he chooses too and he has already.
