Hey all, I’ve recently got a new job that I’m actually happy with, 4/10s and good pay, treated right. BUT my insurance doesn’t kick in until June 1st and I’ve had a hernia acquired in the military that was not necessary to be operated on for years but it finally is, anyway they’re requiring me to have a return to work form in the mean time until my insurance kicks in I just can’t pay to go to urgent care atm for a note, I was hoping someone here would have some sort of a template or just any recommendations?
