I called off at my job a few times due to adverse side effects to medication and now they’re making it so hard for me to return to my normal schedule

I called off work at my job a few times due to adverse reactions to a medication change and they didn’t let me return until I had a release back to work from my dr. And once I did come back they wanted all this paperwork to release my medical records to them so they could determine if I should even be working there with whatever meds I’m on or condition I have. The paperwork was ridiculous and asked how many “flare ups” are expected each week and my doctor literally didn’t know, there’s no duration. So they just wrote 2 days per week. So today I get a form that asks me to sign to either go to an “on-call/back up position” or to resign. And I feel like this place is making it hard as fuck to return to work normal. And the HR lady was an asshole…
