Need advice on what to do about a denied time off

So I put in a UNPAID time off request two months in advance for a surgery I had scheduled. My job has a point system, and I have 0 points and have not called off in over a year o I strategically saved my points for this moment. I talked to my manager before I made the request and told him about it and that I will work gladly holidays after that. He denies the request and it was only for a week (definitely won’t be healed by then) so I put in another request for 6 days, Deny. Then put in another one for 4 days, DENY. I found out tonight that another coworker who got the same surgery was approved 3 weeks off! Im furious as to why hers was approved and not mine when I talked to him about it. Now I have sacrifice my points for my recovery, but I feel it’s really unfair how my time was denied and she was allowed almost a month off. I have not talked to my manager about this yet but feel I will be retaliated if I do. If I go to HR, they will most definitely tell him. What advice do you guys have on the proper way to confront this? The surgery is set and I cannot change the date.
