Remote Work as an Accommodation?

I took a new job in November in part because it was fully remote after training. Now they're making us sign a contract stating we're hybrid and expected in the office two days a week. Never mind I can't afford to commute as we're just scraping by, but this job triggered a major mental health crisis for me in January, and I'm still recovering from the psychosis I experienced brought on by severe stress. WFH is the only reason why I'm able to go to work as much as I do, and even then, I'm counting down the days until I qualify for FMLA. The office environment will undoubtedly cause me to spiral. Does anyone have experience seeking permanent remote work as an ADA accommodation? This seems like the best option for me, short of finding a different iob.
