Probably will get fired due to long sick leave absence (depression). How to approach when looking for new employer?

TL;DR: I am on sick leave for about 3 months now due to depression. Upon returning to work, I will probably be fired. When asked about previous employment in new interviews, how would you approach this? LONG STORY: So I have had major depression diagnosed for about 7 years now. Just heard from a colleague that there is a good chance I might be fired upon eventually returning to work.
I have been in this company for about a year and was considered to have good performance, and received a 10% raise prior to the current issues. Personally, I am not that attached to this job since my department (maintenance) is heavily overworked and understaffed, and the whole company structure is hectic and chaotic. My team and the department manager are great and I will be sad to leave them. The pay is OK for where I live, but nothing special.

Does anyone have advice on how to approach the interviews when asked about previous employment?
Most obvious answer would be to keep my health for myself. How would the questions then be deflected?
On other hand, does anyone have good experience with being upfront with the employer? I know this is not popular, but I personally work most comfortable when I don't have to hide that I have depression issues and can talk openly when I have variations, and we can all prepare if things go south for me. This approach also actually helps me not have burnouts or being pulled into an episode.

Also, I am in south-central Europe (Croatia), for any comments that might include any legal connotations.
