“As you all may know our medical insurance renewal is approaching (12-1-23)and we will be switching insurance providers (I attached a simple plan overview to this email) but in order to get you the new rates the insurance company requires that all full time employees complete the questionnaire thru the link below (if you are part time please disregard). Even if you do not have insurance thru us currently you are still required to fill out the form! This is crucial to getting competitive rates for 2024 therefore if you know of any employees that do not check their company emails very often please let them know to do so ASAP!
[Company Name] is committed to offering all full time employees the most competitive benefits package possible for 2024. We need your help this year to help us accomplish this goal.
We are asking all full time employees to follow the link below and complete a confidential Medical Questionnaire by the end of Tuesday 10-31-23:

When you click on the link you will be asked to put in your personal information – include the company name and then answer a series of Health Questions. Please note that the questionnaires only refer to anything that has occurred in the past three (3) years for yourself or your dependents.
Completion does not commit you to enrollment into the medical plan – this is a data gathering exercise only – therefore, please include complete information for yourself and any dependents that you may consider enrolling in coverage.
To reiterate, this is MANDATORY for all full time employee to complete by end of Tuesday 10-31-23.

***WTF even is this malarkey? It can’t be legal to mandate this personal health data.

TLDR: company states giving away very personal health data is MANDATORY for big health insurance change. Legal?
