When you click on the link you will be asked to put in your personal information – include the company name and then answer a series of Health Questions. Please note that the questionnaires only refer to anything that has occurred in the past three (3) years for yourself or your dependents.

Completion does not commit you to enrollment into the medical plan – this is a data gathering exercise only – therefore, please include complete information for yourself and any dependents that you may consider enrolling in coverage.

To reiterate, this is MANDATORY for all full time employee to complete by end of Tuesday 10-31-23.

***WTF even is this malarkey? It can’t be legal to mandate this personal health data.
Anyone with insight plz help!

TLDR: company states giving away very personal health data is MANDATORY for big health insurance change. Legal?
