Short Term Leave – Do not qualify for FMLA (I do have Aflac disability coverage)

My psychiatrist is recommending medical leave for mental health. I have been in my position with this company since May. I do not qualify for FMLA, but my Aflac policy says I qualify for up to 24 months of leave with full pay. Is my employer still required to hold my job while I’m on medical leave? How can I approach this with HR? Do I need to be cautious when talking to Aflac?
