Manager retaliating after 2 months medical leave

I was struggling with addiction and went to rehab. I got out on 07/29, my manager has been absolutely horrible to me regarding my return. First, I had filled out the wrong form to request a leave and she emailed me telling me I was now on a corrective action plan. On 07/01, I told her I’d be extending my leave. I was admitted to a residential rehab on 07/02. I was in contact with HR through my case manager throughout my stay in rehab. On 07/30 (typical start to my work week) she emailed me and asked me why I didn’t return to work… I told her that HR had informed me that I needed a Return to Work form filled out prior to my return, and since I had just gotten out of residential treatment, I had an appointment in two days on 07/31 to obtain that. First, I had filled out the wrong form to request a leave and she emailed me telling me I was now on a corrective action plan.

On 07/01, I told her I’d be extending my leave. I was admitted to a residential rehab on 07/02. I was in contact with HR through my case manager throughout my stay in rehab.

On 07/30 (typical start to my work week) she emailed me and asked me why I didn’t return to work… I told her that HR had informed me that I needed a Return to Work form filled out prior to my return, and since I had just gotten out of residential treatment, I had an appointment in two days on 07/31 to obtain that. She sent this email at 4:30 pm on a Friday, for the record.

I called the Leave Management Department and informed them that I’d be returning on 08/06, and my doctor submitted my Return to Work form reflecting that return date.

This morning 08/06, I tried to log on and was unable to get into the computer. I called IT and they told me that I needed my supervisor to submit a ticket. I was contacting my supervisor for roughly an hour with no responses, then when she did respond, she came at me for not contacting her to let her know my return to work date.

Then, I sent a screenshot of the email I got from HR, where they told me to contact the Leave Dept regarding the leave & return to work and did not mention once anything about needing to contact my supervisor directly.

I will be contacting her manager, and I will be requesting to change teams. Fuck this
