Long story shortened. I ended up having major surgery in February of this year. Got cleared to go back to work in May. I took return to work paperwork I had to go see my doctor 4x to get new paperwork filled out Everytime cause my boss kept misplacing it on his desk. After the 4th time of him losing it I contacted HR. They asked for details. I gave them everything they asked. They sent me new paperwork. I got it filled out and my doctor faxed it to them. HR told me they had received it and would've approved it on my original return to work date. They then told me to file an ethics complaint with their ethics committee to investigate. Went by my old place of work to pickup a script for one of my kids and was told by the pharmacy tech that store manager was let go and the regional manager had to take over.
