Employer asking for medical diagnosis? Is this even legal?

My new job says that I am expected to go to medical appointments on my time off and if I miss work because of being sick, it will be deducted from my PTO. I must must request authorization within 30 days in advance of a medical appointment and I have to consult with HR if I can't comply with the appointment so we can come up with a better day for that. They require a medical certificate saying the medical condition and its DSM-V or ICD-10 code with reasons why the medical condition prevents the me from working. I feel like that's intrusive? They also say it's not appropriate for me to have a family member call HR if I end up in the ER and can't call. I don't know. I haven't had these weird policies in the past. Is this the norm?
