A staff member was murdered at a mental health facility in the company I work under. My question if they are in violation of some code that an entity like osha etc could be notified to hold accountable and repair the facility?

I am at a different site but I have heard there are still blood stains in multiple areas of carpet etc from where a staff member bled out after being stabbed by a resident while staffed alone. That was three weeks ago now. Not going to post too many identifying details but it’s a very large organization. But what agency in the US, if any, can be contacted to address this? Staff and residence have to see this reminder everyday and certainly there regulations for this sort of thing.
