Boss encourages longer breaks when the workplace is not busy

I work at a clinic that operates from 8-12 and 13-17. Employees get a 1-hour break from 12-13. When the clinic isn’t busy, the manager tells us to clock out early (and take a longer lunch break). I clock out when she asks but clock back in an hour later since I want to work the full 8 hours. I normally just clean the kitchen or rooms since the clinic doesn’t open yet. What can I say if the boss pulls me aside for clocking in when there are no patients?
