Company didn’t offer insurance paperwork

I noticed the other day my insurance said “waived” on my statements which I never did. After asking a few coworkers, they said the same thing and after taking it up to a supervisor, literally means we waived our insurance when we started. There was no paper or email given to me about insurance and all my coworkers say the same thing who started after the open enrollment period. Is there any course of action I/we can take? It’s a newer company so I don’t want to assume it was intentional, but in my eyes a company purposely withholding insurance forms for multiple employees so they don’t have to pay their portion would be a huge ethical issue and potentially a law suit?
