counterproductive/counterintuitive

In a job interview, is it frowned upon to ask for examples of counterproductivity or counterintuition? I have been at two companies, that have had various examples of it, since I started working full time from 2011, I can't be the only person who experiences things like Someone important get's fired/leaves essentially creating a “broken link in the chain” there ends up never being a way to fix the link, so you either circumvent it or circumnavigate it. Not being able to access a room/floor even though it has equipment you require to do your job Requiring to meet goals of international quality control and not paying the bill to allow international couriers and these are just a few.
