Employer denies vacation leading to loss of PTO

Actually not me but a close friend. So he does not accrue PTO every pay period; it seems like he is given X weeks every year on the anniversary of his hire date (October). There seems to be some sort of schedule ie if you've 3-5 years of service you get 3 weeks. Anyway, his company does not roll over PTO but will pay out a maximum of one week. He has approximately 4 weeks of paid leave. He has submitted his vacation request to burn days before he loses his PTO. Somehow company isn't willing to pay out his one week so he has to burn basically a whole month before October 20th. Each and every one of his requests have been denied by his supervisor and there isn't much of an HR he can contact. He works in West Virginia. What can he do?
