How come when workers make a mistake, they’re “held to the highest of standards”, but when management makes a mistake “accidents are sometimes unavoidable”

We had management make a really bad decision. We're probably going to get fined like $250,000. Yet no one in the management team will get in trouble. No repricusions. But if a lowlevel employee cost the company that much, they'd be fucking toast.
