HR won’t accept workplace accommodation unless I get the accommodation letter signed

I am trying to get workplace accommodation because I have PTSD, OCD, anxiety and depression. HR wants me to get a accommodation letter signed even though I told them I have the conditions already and stated what I would want the accommodations to be (part time or flexible breaks).. My family doctor won’t sign any of the accommodation letter and told me I need to go to a Mental Health specialist to have it signed.. I only go through Teladoc and Teladoc can’t sign accommodation letters so when I tried calling around for Mental Health providers around my area, all of them said they are booked until summer 2023.. I emailed HR to ask them what my options are because everywhere is booked and they just sent me employee resources on how to better cope with stress and mental health..
