I lied on my CV, will I fail a background check and can I recover?

I'm having a bit of a panic attack all of a sudden and just wondering if this sub can help me. I got offered a job as a support worker (entry level but experience preferred). On the CV i sent prior to the interview, two employment dates aren't completely correct. When I was at uni last year, I worked as a bank staff at a nursing home very short term, I think between august-october. After that, I stopped asking for shifts as I had my university work placement. My bank contract was officially terminated 6 months after my last shift, so I said on my CV that I was employed there August-April. Another thing, back in 2018, I worked in a nursing home for 2 years. Then I stopped for a year when I started university, then got a summer job in another nursing home the following year. However, on my CV, I just said I was employed with the first nursing home for 3 years as I thought it sounded better (I know, I know).

For my references, I was hoping to put my university work placement (it was unpaid but relevant to my new job) and maybe the nursing home I was at last summer for a basic employment reference (although I'm worried about the dates). I could also put my old boss at the first nursing home as a character reference but she hasn't worked there for years.

How screwed am I when it comes to the HR pre employment check? For some reason I thought it wasn't a big deal to lie a bit on your CV especially when applying to relatively low salary jobs but now I'm concerned. Thanks

Edit: if called out on the information, I might say I thought the dates on the last nursing home were correct as i was still technically employed as bank till April. And I might say I got the year wrong for my end date at the last nursing home. I'm not sure if this would work :S
