I need Short term disability but i dont know how to get it

I am in the US in Colorado and am trying to get into outpatient programs for my mental health. its very bad to say the least. Work is a huge stressor and I cannot mentally do it right now especally if I get into an outpatient program. I know STD gets denied all the time, does anyone have any recommendations for how to phrase my request for STD?
