I think my manager tricked me into signing off on an unprofessional 5mo review

I’m 20 and this is my first proper job. I work as a receptionist in a medical clinic. She called me in for my 5 month review and told me I hadn’t been progressing as they wanted me to. I was taken aback because absolutely no issues were ever brought up to me and I had no idea I wasn’t holding up to standard. We spoke and she was understanding, I explained myself for any issues she brought up and apologised profusely honestly I was extremely embarrassed and confused. I read the “review”. It was the most unprofessional document ever and tbh I cried reading it. Here are some direct quotes: “You act like you know everything” “Get angry when told to do something” “You need to get off your chair and do something” “got very emotional during review” reading this I was heartbroken. I am an extremely timid person and this stuff just simply isn’t true – every time I’m taught something new I write it down, I absolutely am never angry when told to do something. There was nothing positive on the review at all but I’ve been commended for my work by the doctors and even the directors many different times. My manager mentioned that some of this was from “co-worker opinion” and I should mention it’s a very toxic work environment. Most of my co-workers (other receptionists) are older women that have verbally abused me in front of patients and are notorious in the workplace for being rude to new staff but nothing has ever been done.

I showed many people this document and they were appalled, told me to lawyer up, to join a union etc etc but I don’t know if they’re biased. Am I overreacting?
