Panic Attacks due to work related stress

Hello, I don’t know if this is the right place to ask for this but, I recently have had extremely bad panic attacks over the past week, I have been at my current job for only about 8 months, went from part time work for years to a 9 hour shift 5 days a week, and I think it has absolutely destroyed me. I am introverted and have been in a role of constant human interaction, pushing me to a point where on my days off I just lay in bed to try to regenerate my mental energy, but its been strained beyond relief it seems. I just had my second major I think panic attack. I wake up around 1 am from sleep heart racing sweating all over it feels like I am being electrocuted and also being dragged down. I just had my second major I think panic attack. I wake up around 1 am from sleep heart racing sweating all over it feels like I am being electrocuted and also being dragged down. Then I get extremely intense chills as my body is confused trying to regulate my temperature.

The first one caused me to get sick and I have been sick since Thursday 10/12 when that one happened at 1 am.

I guess what I’m asking is, I wanted to put in my two weeks as I could tell my job was the major stressor, but I honestly after this panic attack I don’t think I could last working the next 2 weeks, I am fully able to survive off my savings for at least 3 months before it becomes a problem, but I don’t want to ruin my relationship with my work by leaving on bad terms, so I was wondering if anyone has experienced anything like this and has any advice on how to approach the situation.

I guess I could attempt to get a doctors note? Or maybe I will just have to burn a bridge and if my next employer asks questions I will say it was a gap due to medical complications.
