Pre interview questionnaire asked if I was attractive

I interviewed at an orthodontist office with a bunch of Drs and the questionnaire I filled out first asked if I was attractive and fashionable, I thought it was really weird, because why would that be necessary at an orthodontist office, I didn’t get the job, but I feel I was discriminated against because I wasn’t cute enough to the Dr. What the actual fuck
