Work refusing to follow doctor’s orders, and not paying me the raise they promised a month ago. I need this job to keep the insurance paying for my operation in October, but I’m over them taking advantage of me. What options do I have ?

First time poster. Not sure if this is the right place, but worth a try. ​ I'm at my threshold with the place I work, and need to know my options. A little background: ​ I been working in kitchens over 15 years. I been at my current place of employment [non-profit Private Golf Club in PA] for just under a year and a half. Around December, I was diagnosed with Long Covid and went out on disability for about 6 months. This period of time basically wiped my savings, in addition to multiple bills I still need to pay, leaving me with no option but to return on a 'part-time' basis (I still have yet to be cleared from the Long Covid). My Doctor's Release specifically says MAX 3 days per week, 6 hours MAX per day. Safe to say, I have yet to work one day under 6…

Around December, I was diagnosed with Long Covid and went out on disability for about 6 months. This period of time basically wiped my savings, in addition to multiple bills I still need to pay, leaving me with no option but to return on a 'part-time' basis (I still have yet to be cleared from the Long Covid). My Doctor's Release specifically says MAX 3 days per week, 6 hours MAX per day. Safe to say, I have yet to work one day under 6 hours, and I'm beyond over having to explain my health situation EVERY WEEK just to explain why I'm irritated there's no relief after 6, sometimes 7 hours, and how they can never follow my Doctor's restrictions. On top of that now, the raise my GM and Chef both agreed upon for me to receive two pay periods ago [+$2/hr promised a month ago] , still has yet to show up on my paycheck, let alone the backpay they still owe me. After my last paycheck, I confronted my chef, who confronted the GM, who said it would be fixed on this one. Well, its not. I have a very grungy attitude towards this place, because they had a supplemental insurance agreement through AFLAC that they never informed me about. This supplementary insurance would of paid for all my doctor's visits and tests, and I would not be struggling as hard financially as I am currently, and now I'm no longer qualified for this assistance, as now all my conditions are ' pre-existing'. I know there is not likely anything I can do for that part, but as for my additional promised pay, and ESPCIALLY for the Doctor Restrictions, there has to be some 'illegal' behavior present here, and I need to have my next steps ready in case they try to withdraw my raise after another confrontation. The only problem is, I cannot walk away from this job just yet, as I have an operation in October tied to my current insurance, and need to be insured at that time to prevent the whole operation's cost falling on me. So what options do I have in the meantime?
