Medical Premiums

I’m currently on short term disability. I received a letter from my HR/Payroll department that was basically a collections letter stating I have to pay for my premiums or there “might” be a lapse in my coverage? Its $200 (so far) & I obviously don’t have that kind of money because I’m only getting a percentage of my income. Is this even allowed? It just seems so odd. If there’s any laws against this, for reference I live in KY & work in OH.
