My job has my HR approved breaks noted as out of adherence and it’s impacting my scorecard

There are some other areas I'm “struggling” in, but I was recently put on a verbal warning for my performance. I noticed while we were going over adherence, my HR approved breaks (for a doctor noted medical condition) were showing as out of adherence further bringing my score down. Am I reading too much into this or should I be concerned?
