Mistakes at workplace

Needed advise as to how to respond when stakeholders wrongly blame you for messing up when in fact it was not your mistake or could not have helped in that situation. Do you defend yourself? Note that this is a constant thing and becomes frustrating to constantly defend yourself to highlight to your direct manager that it is not a performance issue. This stakeholder is at a senior level.
